For Health Professionals

How to Refer:

We require a referral letter from a physician, nurse practitioner or other regulated health professional. 

We accept referrals from across Canada. Please note that patients need to attend the clinic in person for an assessment.

Please include details of your patient’s injury (if applicable), a complete medical history, including surgical history, a medication list, current functional limitations and treatments administered.

For patients being referred for nerve pain, please include any relevant imaging reports such as ultrasound or MRI.
